First time posting …
My niece took her baby girl (2 months) to her vaccination appointment yesterday but decided not to have them last minute. The nurse tried to convince her & her partner to go ahead with them and then said you need to sign a form if you’re refusing them.
Does anyone know what the form is that they want signed??

It’s just to say you refused if baby does get ill from one of the illness the vaccine prevents it protects the docs and nurses and you can’t bitch at them for it. Make sure you sign it. It also lets schools and things know in future
The same sort of paperwork that will come all the way through school years – you either sign your consent or sign your refusal (as the child gets older they will also ask for their verbal consent to proceed)
It will just be to show in the records that they refused & they understand the risks of not vaccinating. It’s just documenting their decision
